Das Krankheitsbild des flexiblen kindlich-jugendlichen Knickplattfu es wird in seiner Abgrenzung zur physiologischen Fu senkung im Kleinkindes- und frühen Schulalter sowie mit Überlegungen zur Spätprognose dargestellt. Differentialdiagnostik, Pathomorphologie und bildgebende Diagnostik zur Quantifizierung des Fehlstellungsausma es werden näher erläutert. Der Schwerpunkt liegt auf der Indikation zur operativen Behandlung und der Differentialindikation. Im Lebensalter von etwa 9 bis 12 Jahren kann eine minimalinvasive Arthrorise durch Calcaneostop-Schraube oder Sinus-tarsi-Implantat als wachstumslenkende Ma nahme in schweren Fällen empfohlen werden. Bei älteren Patienten kommt eine Tarsale Triple Osteotomie oder auch die Kombination von Arthrorise und Osteotomien in Betracht. Verkürzungen des M. gastrocnemius bedürften häufig der intramuskulären Verlängerung.
